That is one cute cupcake!
The polka dots might be rolled fondant which would make the whole thing a lot easier to make. Not sure how you would get that perfectly round top though....
That's looking like a frosted styrofoam ball to me! It seems that if the cupcake rose that high, it would have more of a cone top, rather than being so perfectly rounded. Dunno. But I do love the polka dots. . I think they're punched out of rolled fondant. Darling picture!
even if it wasn't rounded just like the photo you could still have a cute polka dot cupcake - yumm
I think you could use an ice cream scoop to make the frosting cute and round, but not perfect.

definitely shape the frosting with something round (like they do with snow cones) and the dots? all it takes is different size icing tips (or you could go to the trouble of fondant dots)!
I suspect it's the result of a very talented food photo stylist. I'd be surprised if that's 100% edible in real life. But it sure is pretty!
An important thing I learned in culinary school was the mixing of muffin and cupcake batter. If the batter is over-mixed you will get "tunneling" in the center, creating a cone near the top of the finished product. If you want a more even, circular top make sure that when you are mixing together the wet and dry ingredients you only softly mix until combined and also mix in a constant, circular motion. It makes such a difference.
As far as those adorable circles go, it looks like they used some sort of gum paste or fondant. If I were you, I wouldn't use fondant because it tastes nasty. However, I have a recipe for fondant using only marshmallows and powdered sugar. It doesn't taste like glue, as normal fondant does, but it is MUCH more messy.
I'm going to be embarrassed if this was a retorical question.

I have seen a baking pan that makes "balls" that are flat on the bottom and rounded on top. I wonder if this is a mix of a standard cupcake with one of this balls on top before frosting? I'd say the polka dots are definitely fondant, though. Good luck!
